I just came across this article in looking for another IG report of VA Denver (2004-a sickening report of the status of the medical center from safety, infectious disease and patient confidentiality and privacy standpoints) and VA in Kansas City (inspection into death of patient that was avoidable, but incompetewnt staff were turfing him all over town until he got to his final resting place (died).)

Neither hospital was in compliance on the main issues years later. As of a followup report, the air system that was to be in place to prevent immunocompromised patients (and employees) from contracting aspergillus infections again were still pending from 2004 in Denver. there had been a previous cluster of patients in 2001 who contracted invasive aspergillosis. the report did not include prior years and did not include other types of aspergillus disease in patients or employees.

The systems issues, involving turf wars and back-biting of staff in the Kansas City case had not been dealt with either, years later.

There is blanket protection for these facilities and their mangement team because of immunity and IG reports that cause any effect are aimed at low level employees, veterans or occasionally contractors.

I am asking my name does not appear in a google search as so often happens with comments
